I recently made one for myself to document all the fun things we do this summer. I wanted it to be small and fit into a bag to take to the beach for instance. Here's what I came up with:

Take an 8 1/2 by 11 inch piece of paper and fold it in half. Open it back up and on that center line, measure and mark 1 1/2 inches from the top and from the bottom. If you have a Crop-A-Dial Big Bite, you can punch your holes.
If you don't have a Crop-A-Dial, fold your paper back in half and use a standard size paper punch to punch your holes. *Be careful doing this. When your paper is folded in half, you only want to punch a half circle on your mark so that when you open the paper, it's a circle. If you punch out more than an half circle, your hole will be more of an oval shape.
